If your Doctor advises that you require a blood sample then you will need to attend a phlebotomy clinic taking your blood test form with you. Please see below for all local clinics who operate a walk-in service except Derby Children’s Hospital where appointments are required. Children under 12 years of age can only be seen at the specific clinics marked*.
